This is what I wrote to get my religious exemption at my workplace.
PART 2 - RELIGIOUS EXEMPTION
Please explain how the COVID-19 vaccination interferes with your free exercise of your religious rights.
"God has willed that man remain under the control of his own decisions. Man's dignity demands that he act according to a knowing and free choice that is personally motivated and prompted from within, not under blind internal impulse nor by mere external pressure." Gaudium et Spes
In some cases, we may need to obtain additional information and/or documentation about your religious practice(s) or beliefs). We may need to discuss the nature of your religious beliefs), practice(s) and accommodation with your religion's spiritual leader (if applicable) or religious scholars to address your request for an exception. If requested, can you provide documentation to support your beliefs and need for accommodation?
